The goal of the Care Navigator is to ensure delivery of optimal care for the patient while promoting fiscal responsibility.
Ensures safe, efficient, cost-effective care by assessing the patient’s clinical, psychosocial, and financial parameters. Develops and implements a patient-centered care plan for the acute health episode that begins with the index hospitalization. Collaborates with the patient, patient representatives, and interdisciplinary team to facilitate optimal outcomes as the patient transitions through the healthcare continuum.
Works in a manner which supports the mission and vision of UW Medicine Northwest Hospital. Ensures that daily operations, procedures and standards of care are maintained in accordance with hospital and The Joint Commission guidelines.
Bachelor of Science in Nursing required. At the discretion of the manager, additional years of relevant experience may be substituted for the degree. Master’s degree preferred.
Three years previous experience in care management is preferred.
Knowledge of medical screening criteria is preferred.
