    Organizes workflow, oversees day-to-day operations, and manages personnel, equipment, and supplies to ensure efficient functioning, quality services, and cost effectiveness of Social Work services within the Care Management Department.  Responsibilities of this section include: the assessment and identification of high risk cases that require discharge planning, support, and/or post acute disposition.   The supervisor continually evaluates resource utilization, length of stay, and appropriate discharge planning criteria for improvement opportunities.   In addition to management activities, the Supervisor provides direct patient care as a Senior Social Worker.






    Education Background

    • At a minimum, a Bachelor of Science in health care field is required. 
    • Degree in nursing or Master of Social Work degree is preferred

    Licensure, Registration, Certification

    • Current Washington State Licensed Social Worker or
    • Current Washington State Licensed Registered Nurse
    • (as relevant to educational background)

    Special Skills/Training

    • Able to demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served on the assigned unit/dept.

    Work Experience

    • Requires a minimum of three years social work or case management experience in a health care setting (preferably an acute care hospital), including utilization management or discharge planning.
    • Minimum of 1 year prior supervisory or lead experience is required.

    Physical Abilities

    • This job requires ability to communicate clearly in English (verbal and written) with all stakeholders (providers, caregivers, patients, family members) for the effective treatment of patients.

    Physical Demand Level:  Sedentary
    In an 8-hour workday, must be able to-

    Lift / Carry 10 pounds occasionally


    As required, must be able to-

    Sit, Stand, Walk, Bend, Squat, Kneel, Crawl, Climb and Reach above shoulders

    Push and Pull

    Perform fine motor functions

    Must possess functional vision, hearing, and speech in order to communicate effectively with clients, client advocates, and staff.
